Authorization/Past Employer Release:
I hereby affirm that the information provided on this application, and any accompanying letter or resume, is true and complete. I authorize investigation of all statements made including contacting past employers regarding my work record. I release/hold harmless and promise not to claim damages from any prior employers for providing information/their opinion of my work history. I understand that misrepresentation or omission of facts called for is cause for dismissal. 

No Definite Period:
Further, I understand and agree that my employment is for no definite period and may, regardless of the date of payment of my wages and salary, be terminated at any time without any previous notice. I understand this application is not a contract. 

Drug/Alcohol Testing:
I agree to submit to any drug or alcohol test that may be required by the employer for my hire, or continued employment at any time. I understand that refusal to take such tests may be cause for denial of employment or termination of employment. 

Background Disclosure Authorization:
I understand that employment or continued employment (if already hired) may be conditioned upon an investigation into criminal conviction records with local, State or Federal law enforcement authorities by a third party investigative company to confirm information I have provided, and I hereby authorize Environment Control of Boise (ECB) to obtain (at any time while my application is being considered or anytime during employment) and rely upon such reports in considering me for employment, reassignment, retention, or discipline. I understand that such reports may contain public record information such as but not limited to driving records, judgments, bankruptcy proceedings, criminal records, and information about my character, reputation, characteristics, mode of living, social security verification, etc. If such report provides information contrary to that which has been provided on this application or any accompanying letter, ECB will, in accordance with the Fair Credit Reporting Act (FCRA), provide a copy of the report ECB has received as well as a written summary of my rights under the FCRA, and give me an opportunity to provide evidence of my statement. I have the right to make a request to the consumer reporting agency (Agency): Verified First, 1550 S. Tech Lane, #200, Meridian, Idaho 83642, 1-888-670-9564 any report furnished to ECB by Agency and to request the nature and substance of all information in its files on me. The Agency will disclose the recipients of any such report on me which Agency previously furnished within 2 years of request.
